Introduction and Purpose

Healthy Dialogues are writing a pharmaceutical needs assessment (PNA) which looks at the health needs of the people who live in your area. As part of the PNA we will be exploring patient and public views of their pharmacy and how they use their pharmacy, using an online survey called the Pharmaceutical Needs Assessment Public Survey.

Healthy Dialogues will always take the necessary steps to ensure that personal data is protected and treated securely. Any data we collect will be held in accordance with the General Data Protection Regulation (GDPR) and the Data Protection Act 2018 (DPA).

No personal-identifiable data is collected in our Pharmaceutical Needs Assessment Public Survey.  We will ask you questions around protected characteristics such as:

  • Age
  • Gender
  • Ethnicity
  • Religion
  • Work-status
  • Sexual orientation
  • Disability
  • Long-term condition
  • Pregnant or breastfeeding

These questions are optional. You do not have to respond to them if you don’t want to. We ask these questions for monitoring purposes and to provide context to your responses. This allows us to make informed and evidence-based decisions.

Why we will use your data:

We are conducting a Pharmaceutical Needs Assessment on behalf of the Health and Wellbeing Board of your council. As part of the Pharmaceutical Needs Assessment, we are exploring people’s views and experiences of their pharmacy and how they use their pharmacy.  

Your answers will in no way affect the service you receive from your pharmacy. You will not be identified in any reporting.

The responses that you provide will help to inform potential decisions on future applications for new pharmacies in your area, or considerations for changes or adaptations at your current pharmacy.

Who will see your data?

Healthy Dialogues team members who are responsible collection and analysis of the data.

How long will your data be kept?

Your data will not be kept longer than necessary. We will delete all data on the 1st of October 2025 at the latest.

How is your data stored and processed?

We use Microsoft Forms, an online survey tool to collect and analyse your responses. Data collected by this method is stored by the providers. To read about their Privacy Policy and Security Statement visit:

You have a right to withdraw consent of processing your personal data at any time. If you wish to withdraw consent, please contact the data protection officer on [email protected]. We may ask you to confirm the approximate date and time you completed the survey so that we can identify your response. Once we have received notification that you have withdrawn your consent, we will no longer process your information you have provided us and will remove and delete your personal data.
